The mission of the Ogden School District Wellness 365 Program is to promote the health and wellness of our employees by inspiring and creating a workplace that will focus on positive health behaviors and celebrate and improve the quality of life for our valued employees.
The Ogden School District Wellness 365 Program will strive to increase the overall health and well-being, as well as productivity of all employees through promotion of all aspects of healthy living. The program seeks to increase awareness of positive health behaviors, provide educational opportunities in a variety of health-related issues and support each employee as they make and sustain healthy lifestyle choices.

Wellness 365 Program Requirements
In order to qualify for the wellness premiums during the 2021-2022 insurance year, insurance eligible employees will need to complete the outlined requirements September 1, 2020 through April 30, 2021. This program is voluntary.
- Complete a Health Risk Assessment. The link to the HRA can be found below.
- Complete a biometric screening through either on-site screenings that will be held on October 19-21 and also in February (dates TBD), or by visiting your own physician. The physician form can be found below.
- Complete at least one preventative screening examination (yearly physical, skin cancer screening, dental, vision, mammogram, etc.).
- Participation in the Trauma Stewardship. This can be done by reading the book on your own or by joining in the book club discussions held September through December. A reflection activity will be provided that must be completed in order to receive credit. Retirees will not need to complete this activity, there will an alternate challenge/activity announced in January.
The information from your HRA and the results from your screening will be used to provide you with information to help you understand your current health and potential risks. Your personally identifiable health information that is provided in connection with the wellness program is private and will never be provided to your supervisors or managers and will never be used to make decisions regarding your employment. The District will only use aggregate information to design wellness programs based on identified health risks in the workplace. Ogden School District will never disclose any of your personal information publicly. You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
